Tennessee Chicken, a takeaway at 276 Lee High Road, London was given the score after assessment on August 3, the Food Standards Agency's website shows.
It means that of Lewisham's 355 takeaways with ratings, 117 (33%) have ratings of five and just two have zero ratings.
