St james plaice fish bar, a takeaway at 150, St Jamess Road, London was given the score after assessment on February 6, the Food Standards Agency's website shows.
It means that of Southwark's 463 takeaways with ratings, 248 (54%) have ratings of five and just one has a zero rating.
